Day Services

Rise Adult Day, Maplewood: Our Maplewood Adult Day program is designed to meet the needs and interests of adults who no longer want to work full time, but would still enjoy remaining an active part of their community. This program includes recreational, social and creative activities, as well as fitness activities, community activities, art and music therapy and educational groups, among other things. The program has an emphasis on the community and being a part of the Maplewood Community Center Community.  We are able to utilize the many exercise activities and workout equipment within the Community Center. This program has a great emphasis on wellness and exercise utilizing the track, exercise machines and workout rooms.  There is also a pool that is available for exercise..  This program typically holds a 1:5 ratio and has a certified brain injury specialist on staff. This program is usually funded by a Medicaid waiver, MSHO, Veteran's administration, worker's compensation, or self-pay.

Rise Adult Day, Mounds View: Rise Adult Day, Mounds View -- Our Mounds View Adult Day program is designed to meet the needs and interests of adults who no longer want to work full time, but would still enjoy remaining an active part of their community. This program includes recreational, social and creative activities, as well as fitness activities, community service, art and music therapy and educational groups, among other things. The program has an emphasis on the community and being a part of the Mounds View Community Center Community.  We are able to utilize the many exercise activities and workout equipment within the Community Center. This program focus is sensory activities for those in the program. This program typically holds a 1:3 ratio and is usually funded by a Medicaid waiver, MSHO, Veteran's administration, worker's compensation, or self-pay.

Rise Adult Day, New Brighton: Our New Brighton Adult Day Location is now open and provides many of the same programming options available at our other Adult Day sites. Like those other sites, this program is designed to meet the needs and interests of people with physical, intellectual, or developmental disabilities who can't or don't wish to work full time, but still would like to remain an active and vital part of their community. This program offers recreational, social and creative activities, as well as fitness activities, community service, art and music therapy and educational groups, among other things. The program has an emphasis on the community and being a part of the New Brighton Community Center Community.  We are able to utilize the many exercise activities and workout equipment within the Community Center.   This program typically holds a 1:5 ratio and is usually funded by a Medicaid waiver, MSHO, Veteran's administration, worker's compensation, or self-pay.

 

Rise Adult Day, Fridley: Our Fridley Adult Day program is designed to meet the needs and interests of adults who no longer want to work full time, but would still enjoy remaining an active part of their community. This program includes recreational, social and creative activities, as well as fitness activities, community activities, art and music therapy and educational groups, among other things. The program has an emphasis on the community and being a part of the Fridley Community Center Community. The Fridley Community Center is a busy place that includes the Fridley School daycare which provides opportunities for those in the program to volunteer if they desire. This program typically holds a 1:5 ratio and has a certified brain injury specialist on staff. This program is usually funded by a Medicaid waiver, MSHO, Veteran's administration, worker's compensation, or self-pay.

 

Rise Adult Day, Coon Rapids: Our Coon Rapids Adult Day program is designed to meet the needs and interests of adults who no longer want to work full time, but would still enjoy remaining an active part of their community. This program includes recreational, social and creative activities, as well as fitness activities, community activities, art and music therapy and educational groups, among other things. The program has an emphasis on the community and being a part of the Emma B Howe YMCA Community.  We are able to utilize the many exercise activities, classes and workout equipment within the YMCA.  There is also a pool that is available for exercise.  This program typically holds a 1:5 ratio. This program is usually funded by Medicaid waivers, MSHO, Veteran's administration, worker's compensation, or self-pay.

Mental Health Supports

Adult Rehabilitative Mental Health Services (ARMHS): This program is intended to serve those 18 years or older who have a primary diagnosis of a serious mental illness. They must also have Medical Assistance (MA) or Prepaid Medical Assistance (PMAP) coverage. We develop personalized plans for each individual and help guide the person toward specific objectives and goals. The program is intended to promote independent living and help reduce the risk of relapse and mental health hospitalizations for people with mental illnesses.

Housing

Mental Health Housing Support Services and Rapid Re-Housing: This program assists people in making a smooth transition into living more independently in their community through transitional housing and support services. People in this program must have a diagnosis of a Serious Mental Illness (SMI) and be able to live independently in their community without supervision, but with support services. They must also be seeking services in Pine, Isanti, Kanabec, Chisago, or Mille Lacs counties in order to be eligible. Typically, this program is funded by the Region 7E Adult Mental Initiative or HSASMI funds.
