This program serves individuals who…
- Are 18 years of age or older.
- Possess a felony level conviction.
- Are releasing into Benton, Sherburn, Stearns, or Wright County.
- Need additional re-entry community coordination and supports, and/or who benefit from a higher level of assistance.
Program Activities
Service Coordination and planning with other transitional team members including case managers and supervising agents; including but not limited to…
- Application assistance to various programs (housing, employment, ARMHS)
- Aid and benefit application assistance (medical assistance, SSI/SSDI and cash assistance)
- Accessing identification materials (driver’s license, state identification cards, birth certificate, social security card)
- Community resource-based referrals (medical/mental health services, support groups)
Referral Information
Referrals must be sent to Rise via mail, email, or fax. Referrals must include…
- Completed Rise application
- Signed release forms for all parties that require communication
- General introduction to the individual being served
- Location of incarceration and current visitor policy
If available, we also ask that you send…
- Charge information
- Release conditions and information (Date, time, county)
- Individual community support plan
- Most recent psychological evaluation (Diagnostic assessment)
- Most recent treatment records
- Services received within the facility
- Information considered pertinent to placement services
- Pre-established release plans
